How Does it Work?
Fill out a Request for Mediation form
Fill out a Request for Mediation form. The form will identify the parties wishing to mediate, provide a description of the dispute, and let us know if the parties have an agreement to mediate. A $100.00 administrative fee will be incurred at this time.
Mediator Selection Process
If the parties have an agreement to mediate, FORUM will locate three or more qualified mediators to mediate the dispute. The parties may agree to a mediator on the list, find a mediator they would like to use independent of the list, or, if they are unable to agree, strike and rank the mediators provided by FORUM.
Schedule Mediation
Once the mediator is selected, FORUM works with the parties and the mediator to schedule the mediation and collect a deposit based on the anticipated duration of the mediation and the mediator’s hourly rate.
Resolution
Once the mediation is scheduled, FORUM assists the mediator in collecting any additional information needed for the mediation and coordinates the mediation itself. If the mediation is held virtually, FORUM has the capability to run the virtual mediation and move parties between breakout rooms at the request of the mediator.
